Seattle Delivery Partners can use PSST for any of the following reasons: (i) for a personal mental or physical illness, injury, or health condition; to accommodate the Seattle Delivery Partner's need for medical diagnosis, care, or treatment of a mental or physical illness, injury, or health condition; or a Seattle Delivery Partner’s need for preventive medical care; (ii) for care of a family member with a mental or physical illness, injury, or health condition; care of a family member who needs medical diagnosis, care, or treatment of a mental or physical illness, injury, or health condition; or care of a family member who needs preventive medical care; (iii) if Grubhub has suspended or otherwise discontinued operations by order of a public official, for any health-related reason, to limit exposure to an infectious agent, biological toxin, or hazardous material; (iv) if Grubhub has reduced, suspended, or otherwise discontinued operations for any health- or safety-related reason; (v) when the Seattle Delivery Partner’s family member's school or place of care has been closed; or (vi) for a number of reasons related to domestic violence, sexual assault, or stalking.

I agree that Grubhub may collect and process personal data, which may include health information, submitted by me or on my behalf to verify my eligibility for the driver support pay program or PSST and as may be required by law.

I agree that such personal data is not being collected or used for purposes of obtaining or receiving healthcare from or on behalf of Grubhub, and that the collection and use of this information does not reflect or form an employment relationship between me and Grubhub.

I understand that a ‘shelter in place’ order announced for my area does not impact food delivery and that alone does not make me eligible for driver support pay
